The Bali Tourist Levy applies per entry to Bali. Once paid, it is valid for that specific visit and does not cover multiple entries. If you leave Bali and return, you are generally required to pay the levy again.

It is important to understand that the validity of the levy is separate from your visa or permitted length of stay in Indonesia. The levy only covers the tourism fee requirement and does not determine how long you may remain in Bali or Indonesia.
